Dopo che - nelle vicende narrate nel secondo episodio della serie The Eternal Story, inedito in sala in Italia - Madoka si era sacrificata, pur di eliminare le Streghe e riscrivere le Leggi dell'Universo, nella città di Mitakihara sembra che nulla di tutto ciò sia mai accaduto e le ragazze magiche si ritrovano nuovamente insieme a sconfiggere i malefici Nightmares.
