As seen here on an archived version of The CW's website, The Book of Ruin Chapter 4 was the only episode to have a subtitle. The others did not.

I have updated the source in the original message because it was no longer accessible.
As seen in the original message's source (https://static.wikia.nocookie.net/arrow/images/0/06/The_Book_of_Ruin-_Chapter_One.png), the title card in the episode includes the subtitle.
There have been other reports in the past where the title would be written one way in the original source, but in the episode's title card, the name would be different. In these cases, the title in the episode would be favoured over the one in the original source.
