She leaves during season four never to be seen again. Did Polly want off the show or did they fire her?

I don't know the details... but I think her character became very popular and they gave her own show called "Flo". But I think it failed, or else you would know about that show's existence, I don't know how long it lasted. Go check it out... (somewhere in Google Land).
