The pageant was held in Wichita, Kansas, for the fourth and last consecutive year, and was hosted by Dick Clark for the final time, with color commentary by Leeza Gibbons and Courtney Gibbs, Miss USA 1988.

This was also the last time the State Costume competition took place and would not return until twenty-eight years later in 2021, under a different organization.

The pageant was hosted by Bob Goen for the first of three years replacing Dick Clark, with color commentary by Arthel Neville and a special guest appearance by Laura Harring, Miss USA 1985. Entertainment was provided by Doug Stone. During the live broadcast, the organizers of that year's Miss Universe pageant held in Manila, Philippines were introduced during the show along with that country's representative to said international competition, Charlene Bonin Gonzales, Miss Philippines 1994.

Miss USA 2003 was the 52nd Miss USA pageant, held at the San Antonio Municipal Auditorium in San Antonio, Texas on March 24, 2003. It was the first time it had been held in this state since 1996 when the pageant was held in South Padre Island. Beginning with Miss USA 2003, NBC assumed co-ownership of the pageant along with Donald Trump, and as a result began televising the pageant.

Daisy Fuentes co-hosted the event for the first time (although she had been a color commentator in 1995), joined by Billy Bush, who would also host the 2004 and 2005 events, as well as the Miss Universe pageants all three years. Color commentary was added by the reigning Miss USA Shauntay Hinton. Entertainment was provided by Burn The Floor.

The judges included future first lady Melania Knauss, soap-opera star Kristian Alfonso, football player Warren Sapp, actor Mekhi Phifer and former astronaut John Blaha.

Miss USA 2004

Miss USA 2004 was the 53rd Miss USA pageant, held at the Dolby Theatre in Hollywood, California on April 12, 2004.

Pageant co-owner Donald Trump opened the pageant by telling Castillo "You're fired", his catchphrase from his television program, The Apprentice. The pageant was hosted by Billy Bush and Nancy O'Dell. This was the first time this partnership had hosted the pageant. Bush had previously hosted Miss USA 2003 and Miss Universe 2003.

Miss USA 2005 was the 54th Miss USA pageant, held in Baltimore, Maryland on April 11, 2005. Fifty-one state titleholders competed for the title of Miss USA in the Hippodrome Theatre on April 11, 2005. The hosts were Access Hollywood stars Nancy O'Dell and Billy Bush.

Miss USA 2006 was the 55th Miss USA pageant, held at the Royal Farms Arena in Baltimore, Maryland on April 21, 2006.

This was the second consecutive year that the pageant was held in Baltimore, although the venue changed from the Hippodrome Theater to the larger 1st Mariner Arena. Delegates arrived in the city on 2 April 2006, and were involved in three weeks of events, appearances and preliminary competitions before the final competition. This included a trip to New York City to attend a book launch for The Miss Universe Guide to Beauty and make media appearances on Regis and Kathy, The Early Show and Total Request Live.

Miss USA 2007 was the 56th Miss USA pageant, held at Dolby Theatre in Los Angeles, California on March 23, 2007, after two weeks of events and preliminary competition. The pageant was broadcast live on NBC from the Kodak Theatre in Los Angeles, California one month earlier than the 2006 pageant. This was the second time that the pageant was held at this venue; it was previously held there in 2004, when Shandi Finnessey of Missouri was crowned Miss USA 2004. The host hotel was the Wilshire Grand.

He had previously co-hosted Miss USA 2003 with Daisy Fuentes and Miss USA 2004–2005 with Nancy O'Dell. Entertainment was provided by Kevin Rudolf who performed "Let It Rock" during the swimsuit competition and The Veronicas performed "Untouched" during the evening gown competition.[
