What would we have seen and heard that we didn't, or that was only alluded to during the show?
And is it possible to make a pure comedy in cinematic style?

No need for a new set. The show just reuses the set used when Penny and Leonard turned Sheldon's room into a "sex dungeon". With Leonard as a green Orion slave boy.

As Knixon said, there would be fewer seasons and fewer episodes per season. And I'm going to guess as a result that each episode would run for an hour. I like Z's idea of the screen going black at the end of the series with Soft Kitty playing.
Also, there would be 2-3 years between seasons- so long that you forgot what had happened on the previous season by the time the next one got there.
And Penny could curse like a sailor.

Or in the series finale Bob Newhart, aka Professor Proton, could wake up in the bed with his wife from his first TV series. Oh wait, that's not very Netflix-y.

Even though Sheldon was having a nightmare, still surprised he didn't pass out.

I think it would have gone down a far too serious road personally, especially with Howard's parents and Leonard's mother problems. And I do think too cinematic for the almost 'out there' comedy moments.

Yes, I agree with you. The sitcom format allows for the over-the-top and/or almost slapstick comedic scenes, and I think the HBO and Netflix series have a much more serious, in-depth style.

There are plenty of half-hour comedies on cable/etc. Veep for example. It might have been actually closer to half an hour, though. Unlike the less-than-20-minutes TBBT often had.
Veep, considered a great hit etc, had seven seasons, with an average of nine episodes per season. I wouldn't have wished for TBBT to have so little total material. I LIKED it.

More visuals of Howard’s adventures in Astronaut-Land (especially flight school and survival training). Just hearing about them was funny. Actually seeing them, no doubt, would have been hilarious.
As great as his acting and the writing were; still, when it comes to what sounds like some of the “best stuff”… all we have is our imagination.
Really thought this was going to be Howard’s WOW! year nomination and win-wise.

Every episode would start with a different version of the theme song.

But you wouldn't have seen more of that, or anything else. With less than half the number of episodes per season, and maybe half the number of seasons too, we likely would have had only 1/4 of the material we were fortunate to get.
