If Penny is 4th on Sheldon’s friends’ list, and Leonard is 5th; then the 1st, 2nd and 3rd positions must be filled by his imaginary friends.

What Sheldon says in The Bozeman Reaction might contradict that. Don't forget that he also played "Words with Friends" with Stephan Hawking which might make him think that Hawking is his top friend. Also in young Sheldon, Tam is his best friend until he made him an enemy. Who would Sheldon's imaginary friends be ? Wheaton might have been one until he also became an enemy and then a friend again. Friends are something Sheldon can't decide on for the most part.

With the people part being real, and the “friends” part being imaginary; I’d agree on Hawking and add Professor Proton. But, that would still leave the 3rd position vacant—can’t
who that could be.
I guess Sheldon has so few friends that he holds on regardless; and not even death is an impediment when it comes to his friendships.

Sheldon's only worthwhile friend whether he knows it or not is Leonard, then Howard, Raj and Penny. Then he treats them so badly that doesn't deserve any of them.

Or in the episode when Leonard told Penny how he met Sheldon and we see Sheldon's pre-Leonard roommate 's writing on the wall "Die Sheldon Die !"
