What drastic change or event could they introduce that would draw you in for next season that isn't a cheap gimmick , or to avoid using the infamous phrase we all hate : leaping the carnivorous fish?
Maybe Raj wins the 700 million dollar Powerball?

I think an interesting twist would be Amy winning a Nobel prize .Sheldon struggling with his jealousy and expectation to be happy for her could be a direction they go in.
And God forbid Raj have that much money at his disposal. He'd spend most of it on booze and Girls and the rest he would waste.

If someone wins a lottery on the show, it shouldn't be Raj. Now if Mary came into a bunch of money, that could be very interesting. Or maybe...Kripke? Except he has that other show keeping him busy, unless it's not coming back.

Beverly sells the film rights to "Needy Baby Greedy Baby" to Disney and it is made into a full length animated film and then a live action film.

Would Disney make a movie including Leonard's one-side breast fetish, learning to masturbate, etc?

All I know is, if Amy says "no" I'm going to be so po'd! Ideally, I would like to see an emphatic YES that rivals her reaction to the tiara.

If she just says yes, it will be because she's ignoring all those warnings. Warnings from Leonard. From Penny. From everyone. Even warnings from SHELDON.
Which of course doesn't mean that she won't still say yes. Just that it will be stupid if she does. Especially right off the bat. It will still be stupid if she agrees to a long engagement with an "engagement agreement" and at least TRIES to get Sheldon to be a little more normal first. But it might at least be forgivable.

Sheldon wakes up and realizes that the whole kissed-by-Ramona-proposed-to-Amy thing was just a dream.

Now that I could go for. Especially if it turns out Sheldon actually married Ramona, and ESPECIALLY if that happened after Amy turned him down!

I wouldn't be shocked if she initially says no because she's not ready. I know it seems out of character but the writers make her more rational when they need drama in the plot.

A twist I would like is if they made the show as a comedy instead of the soap opera it is.

Stuart gets a life? Put some life in Stuart? Give Stuart back his life?
When the show started he was an artist. If I’m remembering right, Penny was his date for one of his showings (or maybe his only showing?).
Anyway; he is “(re) discovered,” his art becomes sought-after, the $$$ start rolling in.
Would love to see Stuart (for a change) go down a road similar to the one Raj took when he was chosen for "30 Under 30".

Hey, worked for Roseanne, right?
But seriously, this is a tough call. The only good twist I can think of is one that's already been mentioned. Either Penny or Leonard is offered a big position in New York (or wherever) and there's a plotline of should they leave or not. The job can always fall through in the end and they end up staying.
Perhaps they decide to stay because they can't bear to leave Sheldon behind, then Sheldon suddenly turns around and takes a job at Harvard, pissing everyone off.

If Leonard were offered some kind of "Einstein Chair of Physics" at his alma mater Princeton, that would also have the side benefit of making Sheldon very jealous.
Then maybe Leonard and Penny decide they can't leave Sheldon "alone" even with Amy for company, so Leonard declines. Princeton then offers the job to Sheldon who jumps at the opportunity without regard to his friends.

How likely is that, considering:
(1) Sheldon said that Princeton had gone downhill (not his exact words) considering Leonard graduated from there. Sheldon is all about being a Big Fish in what he considers a Big Pond.
(2) It would mean that he is accepting something AFTER Leonard turned it down; thereby acknowledging that he is second-to-Leonard.
