I don't think that will be her reason to break up.

She knows he is lactose intolerant:   "so he can’t eat anything here without his intestines blowing up like a balloon animal."

She doesn't mind him being short:   "Oh, you’re my little homunculus."

She won't be surprised if he becomes bald, as Sheldon has often said something like:   "Don’t forget the male pattern baldness. When his uncles sit around the dinner table they look like a half carton of eggs."

In the past she would stay with him because he paid for things, like food: "Don’t worry about it. It’s my treat."
But now she earns more than he does, so knowing all this: why is she still with him?

I won't be surprised if he is the first who breaks up with her when he starts cashing in with his invention. There will be a lot of young attractive women interested in a successful intelligent man.

At this point, I'd say no. Penny knew all of those thing about him and still insisted on proposing to him. If they break up, it'll be for other reasons. Besides, the issue is not about his current diet, as that has never really changed throughout the show. It's the fact that he's older and the weight doesn't come off.

Penny has either said or agreed that Leonard is cute, good in bed (to an extent) and certainly highly intelligent, but I don't think she's ever thought of him as or described him as sexy. A hot bod is not why she's with him, so unless he became grossly obese, I don't think it would be much of an issue.

I recall two episodes where they were using the word sexy. Of course I don't know if it was just light banter.
