I think the flash forward of the red-eyed Rick was at the end of the season when they capture Negan and he decides not to kill him. This is Rick struggling whether to kill him or not.

Rick made a promise to kill Negan. Not sure why Rick would still be struggling with whether to kill him or not. After everything Negan's done, it's a no brainer. No pun intended, Glenn and Abraham.
