If your going by most people killed directly or indirectly, I'm gonna have to say Littlefinger. He pulled the strings that had Lysa kill Jon Arryn which began the snowball effect to start the war of the five kings.
Indirect kills don't count. No one wants to blame Stalin's mother.
And you guys will wait a long time to see Cersei get "what's coming for her". She'll be there for most of the sequel too.

I still stand by my choice of Littlefinger since his actions/schemes have caused the most deaths in the 7 kingdoms. Also, Stalin's mother did not scheme like Littlefinger did. He purposely caused chaos in order to "climb the ladder".
Regarding Littlefinger, I refuse the idea of making people responsible for more than what they do.
The war was consciously started by an asshole named Ned Stark, not by Littlefinger who clearly advised against it in the very moment the decision was taken, then did all he possibly could to avert it.
