Both titles are valid for the Spanish language.
One for Spain, es-ES, Del revés 2
and the other for Mexico, es-MX, IntensaMente 2
The main poster is from Spain.
To change the main one, just vote on the images with thumbs up or down.
The main one is always the one in the API Request response.

There is currently no way to specify the country for the images. Only the language.
It is part of the plans to separate them by country and not just by language.
