Similar items are built looking at keywords and genres. Recommendations are built by looking at other users who have rated the item and then the rest of everyone's ratings to build more interesting associations. Very, very different approaches.

We used to use similar items but they never really provided a terribly useful list of movies. Everything here is now powered by the recommendations.
