API signup page is broken if the user has zoomed fonts

I'm on a relatively high-DPI display and when I went to sign up for an API key, I zoomed in a couple steps in Chrome so I could better read the text. Once I hit "Accept" at the bottom, I was presented with a form that was quite broken. See this screenshot:

It took me a few minutes to realize that the form was broken because I had zoomed in. Ctrl-0 fixed it right up. Other people might not realize that, especially if they've got fonts zoomed by default. Just an FYI.

Also, the phone number entry box doesn't allow dashes or parens. It forces me to use digits only. That's kinda silly.

Yes, the API request form needs some love. Hopefully we'll find some time for this soon.
