When the configuration changes (which isn't often I agree), Is it likely to be just the base url that changes and the image sizes (w185/w500) etc stay the same?

Yes. We have only updated the
base_url2 times in 3 years, and have no intention on changing it again any time soon. We have never removed a size, only added I think 1 or maybe 2.Hope that helps.

Is this still the case, does the configuration rarely change? Because if it is pretty static it would simplify my client logic greatly.
