Where can I apply for a commercial key?

The information in the FAQ states:
http://www.themoviedb.org/about/api-terms If you want to apply for a commercial API key, go to the following form to see what information you need to provide.
But there's no link provided to this form?

Hi krikkevw,
Just pop into your account page and click the "API" item in the left sidebar. Follow the instructions to create a new API key.

Thanks for the quick reply. I've created a key, but can't select developer or commercial type. Is this type of key also usable for commercial websites? I've also contacted you through the form ('Contact us') in the website footer regarding this 'commercial key'. Can you please provide us feedback about my specific questions which I've submitted via this form?
