API Key Request for movieinfousa

I have a developer api key and was told to re-apply... However, the api key request form is missing as a menu item... if I already have a developer key. Perhaps this is a bug or UX error.
I could create a new account to get the form but am not sure if this is what TMDB would like me to do. Also, I could not post my application here because it was "suspected to be spam."
Thus: please provide link to commercial application form. Thanks.

You can only have 1 API key per account. Who/why were you told to re-apply for a new key?

Ah, I see. Well, you can only have 1 API key per account so if you want a commercial API key you will need to register a second account. You are free to continue using your developer key while you wait for your commercial key to be approved. Hope that explains everything.
