There is another example at the start of Season 4 where there is a "feature length" double episode that is now treated as 2 separate episodes back to back despite being broadcast as 1 episode, this causes no issues with the metadata of the rest of the season. In this case, the "2nd" episode number was skipped at the time, but now exists as 2 episodes.

Yes, and now it is labelled as episode 12 because that is how the episode is now being consumed.
Why must it be consistent with how it was, when that is no longer relevant to how it is currently being consumed.
If someone wants to look something up concerning this particular episode whilst watching it (or the rest of season 3), they are being shown incorrect information that will just confuse them...
Information about quirks of the original broadcast should be stored in separate fields or addendums.

Apart from chapter 11-12, it has a chapter 20 (No choice in duty)

There are 20 episodes only if 11 and 12 are split, which shouldn't be the case.
Otherwise, there are only 19 episodes ("No choice in duty" being episode 19 as visible here).

So in Season 3 why isn't the episode after 11 Siege Protocol, 13 Fog of war? Again, the lack of consistency is the problem.

Because it is numbered as episode 12 by the broadcaster.
We are consistent with what was done by the original broadcaster.
